Addiction and substance abuse disorders, anxiety, depression, and suicidal thoughts, fall under the heading of mental health conditions. The prevalence of mental health concerns in the United States has risen over the past two years due to the COVID-19 pandemic. COVID-related stressors, such as grief, fear, anxiety, and isolation, contribute to the nation’s decline in mental health.

A Centers for Disease Control and Prevention (CDC) survey conducted in June 2020 revealed that almost 41% of adults reported at least one mental health concern due to the pandemic. Of these respondents, 13% started or increased substance use. Alcohol has been the most used drug among seniors, according to the National Institute on Drug Abuse (NIDA).

Data from 2018, prior to the COVID-19 pandemic, indicates that over 1 million older adults live with a substance abuse disorder. Seniors are particularly at risk for addiction and substance abuse problems, not only due to COVID stressors but misuse of medications prescribed to manage chronic pain.

Dual diagnosis, when a person has a mental health condition and a substance abuse disorder at the same time, is common. Almost 40% of adults with substance abuse disorders have a mental illness too. Screenings and treatments for mental health and substance abuse frequently overlap. For instance, counseling and medication management are appropriate treatments for someone living with a dual diagnosis.

Medicare provides coverage for mental health and substance abuse screening and treatment, but the prevalence of substance abuse disorders and access to care and services varies by state. This article includes information about Medicare coverage for mental health and substance abuse, opioid treatment programs, and state resources for alcohol and substance abuse treatment.

Does Medicare Cover Substance Abuse Treatment?

Medicare covers mental health and substance abuse treatment under Original Medicare Parts A (hospital insurance) and B (medical insurance) or under a Medicare Advantage Plan, which provides the same benefits as Original Medicare. Treatment must be deemed medically necessary, that is, needed according to accepted standards of medicine to diagnose or treat your condition.

You must receive services from licensed professionals who agree to charge what Medicare approves for payment. If you have a Medicare Advantage Plan, you must follow your plan’s rules for how you receive substance abuse treatment, including network, referral, and prior authorization requirements.

Medicare also covers prescription medications for substance abuse treatment, but you must have a Medicare Prescription Drug Plan (PDP) or a Medicare Advantage Plan that includes drug coverage. Check to see if you are eligible for financial assistance with prescription drug costs here.

What Types of Substance Abuse Treatment Does Medicare Cover?

Medicare Part B covers preventive screening and limited counseling to help detect and treat substance abuse problems early. Substance abuse can be a chronic concern that gets worse over time if left untreated. As a condition progresses, treatment becomes more intensive and potentially costly:

Part A
  • Inpatient care to diagnose and treat mental health issues either in a general hospital or a psychiatric hospital. Up to 190 days of inpatient psychiatric hospital services during your lifetime.
Part B
  • Alcohol misuse screening once per year and up to four brief face-to-face counseling sessions per year for diagnosed alcohol misuse
  • Up to eight tobacco-use cessation counseling sessions per year if you use tobacco
  • One initial depression screening and review of risk factors annually
  • Individual and group psychotherapy with doctors or certain other licensed professionals are allowed by the state where you get the services.
  • Family counseling if the main purpose is to help with your treatment.
  • Ongoing evaluation and testing to determine if your plan of care for services and treatment is helping you.
  • Psychiatric evaluation.
  • Medication management.
  • Opioid use disorder treatment services
  • Online therapy and counseling sessions (telehealth)
  • Partial hospitalization in a hospital outpatient department or community mental health center
Medicare Advantage (Part C)
  • Must provide all Part A and B services and may include other non-Medicare covered benefits, such as Additional telehealth benefits, additional individual or group sessions, and transportation to or from mental health care services. You must follow your plan’s rules for how to obtain services.
Medigap (Medicare Supplement)
  • Most plans pay for Part A deductibles and all or a portion of Part B coinsurance charges. Medigap is only available to supplement Original Medicare and will only cover Medicare-approved services. Most plans cover an additional year’s worth of inpatient days.

What Kinds of Substance Abuse Treatments Are Not Covered by Medicare?

Medicare doesn’t cover these mental health services, which may be part of a substance abuse treatment plan of care.

  • Transportation or outpatient meals
  • Phone services, or “apps”
  • Nutritional supplemental vitamins and minerals
  • Mind-body therapies: yoga, meditation, exercise, and tai chi
  • Herbal medications
  • Acupuncture
  • Massage
  • Homeopathy
  • Hypnotherapy

Medicare won’t pay for services provided by a licensed healthcare professional who does not accept Medicare assignment. All providers must be eligible by Medicare and state standards.

  • Private duty nursing
  • A phone or television in your room
  • Personal items like toothpaste, socks, or razors
  • A private room, unless medically necessary

Care in a specialized psychiatric hospital is limited to 190 lifetime days.

What Are Your Out-of-Pocket Costs for Medicare Substance Abuse Treatment?

Part A
  • Deductible ($1,556 in 2022) for each 90-day benefit period. No coinsurance for days one to 60.
  • $389 per day for days 61 to 90.
  • $778 for days 91 and beyond, including 60 “lifetime reserve days”
  • If you have exhausted all of your lifetime reserve days, you are responsible for all costs.
  • If you receive care in a specialized psychiatric hospital, you pay all costs after 190 lifetime days.
Part B
  • 20% of the Medicare-approved amount for all Part B services you receive while you are in the hospital, such as doctor’s visits or therapy sessions,and as an outpatient (except for opioid treatment programs)After you pay your annual deductible ($233 in 2022).
  • Monthly premium ($170.10 in 2022 for most people. Will be higher if individual annual income is over $91,000).
Medicare Advantage (Part C)
  • Inpatient services: Daily copay for the first five to seven days for each admission or transfer. Prior authorizations apply.
  • Outpatient services: Copay for prior authorized, in-network services, in addition to the annual health deductible if your plan has one.
  • Copays for Medicare-covered services go towards your maximum out-of-pocket cap.
  • Monthly Part B premium
Medigap (Medicare Supplement)
  • Part B annual deductible ($233 in 2022)
  • Your plan’s monthly premiums

Treatment costs for substance abuse disorders can add up quickly. You may be dually eligible for Medicaid and Medicare to help with costs.

What Is the Medicare SBIRT Program?

The SBIRT (screening, brief intervention, and referral to treatment) program is an approach aimed at catching potential substance abuse disorders early. With Medicare, SBIRT is provided as a Part B service and is billed under G codes. According to the Substance Abuse and Mental Health Services Association (SAMHSA) which is part of the United States Department of Health and Human Services (HHS), SBIRT programs provide:

  • Screening to assess the severity of substance use quickly and identify the appropriate level of treatment.
  • Brief intervention, which focuses on increasing insight and awareness regarding substance use and motivation toward behavioral change.
  • Referral to treatment for those identified as needing more extensive treatment with access to specialty care.

What Opioid Treatment Programs Does Medicare Cover?

Medicare covers opioid use disorder treatment services from opioid treatment program providers enrolled in Medicare. Your health care provider or Medicare Advantage Plan can tell you where to receive services. You can also visit Medicare.gov/talk-to-someone and select “Opioid Treatment Program Services” to find a program near you. Services include:

  • Medication like methadone, buprenorphine, naltrexone, and naloxone
  • Care coordination and case management
  • Counseling and individual and group therapy (in person or virtually using your phone or computer)
  • Drug testing
  • Intake activities
  • Periodic assessments

You pay nothing for these services with Original Medicare Part B, if you get them from an opioid treatment program that is enrolled in Medicare after you’ve met your Part B deductible ($233 in 2022).

You may pay a copay for these services if you have a Medicare Advantage Plan. A deductible will apply if your plan has one. Check with your plan about where you can receive services ― the opioid treatment program must be enrolled in Medicare ― prior authorization requirements, and how much you pay.

If you get your services in a hospital outpatient clinic or hospital outpatient department, you may have to pay an additional copayment or coinsurance amount to the hospital with Original Medicare or a Medicare Advantage Plan.

Knowing where to find and reach out for help are the first steps toward health if you have a substance abuse disorder.

Each state has resources for mental health support, opioid treatment programs enrolled in Medicare, drug and alcohol rehab centers, and help to get naloxone to reverse opioid overdose. Some states have syringe exchange programs through NASEN. State Medicaid programs also provide financial support for substance abuse disorder management.

Expert Tips for Medicare Coverage of Substance Abuse
  • Early detection and prevention are key to staving off chronic problems with drugs and alcohol. Take advantage of screening services available through Medicare and seek mental health care if you think about turning to alcohol or substances when you feel stressed. Mental health concerns and alcohol or substance abuse disorders frequently go hand in hand.
  • Educate yourself about how drug and alcohol abuse develops. Early warning signs include using or misusing your prescription drugs, especially those that are addictive, and using drugs or alcohol to the extreme for recreational or social purposes.
  • If you struggle with alcohol or drug addiction, know that you are not alone. Access the mental health and substance abuse resources available to you through your state’s programs, including Medicaid if you have low income and resources.
  • Make sure your providers and treatment centers are enrolled in Medicare and accept Medicare-approved charges for services. Check with your plan’s benefits administrator about how you obtain services if you are in a Medicare Advantage Plan.

As a health care professional since 1987, Kelly Blackwell has walked alongside and cared for seniors as they journey through the season of their fourth quarter of life. Blackwell holds a Bachelor of Science in nursing from the University of Northern Colorado, a Master of Science in health care administration from Grand Canyon University, an interprofessional graduate certificate in palliative care from the University of Colorado Anschutz Medical Campus and holds a Certified Senior Advisor® credential from the Society of Certified Senior Advisors.

Blackwell contributes to the University of Colorado-Anschutz blog and has been published in “The Human Touch” distributed by the University of Colorado Center for Bioethics and Humanities. She cowrote “Dying Is” for Pathways Hospice.

A registered nurse, Blackwell understands health insurance choices influence quality of life and are driven by values, goals, and beliefs. She’s passionate about engaging with, educating, and empowering seniors as they navigate the health care system. She’s equipped to lend an experienced, compassionate voice to beneficiaries seeking information about Medicare Advantage Plans.
